PET HEALTH PLANS We are happy to launch our new pet health plans. Your dog/cat can join the puppy/kitten plan at any age for 1 year and then go on to the adult plan. When your dog/cat has reached his senior years you will have the option to put him on to the senior plan, this is an enhanced plan that gives you the opportunity to catch things early as it includes full annual blood screening and urineanalysis. * The standard package for an adult cat includes: 1 weight check 1 vaccination for Flu, enteritis and leukemia with full health check, 1 consultation with second health check (claws, anal glands, skin and joints check) 1 health check booklet and protective wallet 12 months' worm and flea treatment, 1 microchip and microchip checks, nutritional and behaviour advise, 10% discount off all procedures, treatments, food and toys * Treatments for a kitten is the standard package with 2 kitten jabs (RCP Leukemia) instead of 1 jab and 1 consultation. This also comes with 1 growth chart. * Treatments for a senior cat is the standard package with 1 blood screen 1 urineanalysis. * Treatment for an outdoor cat is the standard package with 1 blood test for leukemia and cats AIDS ************ 4 payment options are available for your pet health plan: - Plan with Indorex household spray for fleas and house mites. Option 1: discounted single prepayment equivalent to 11 months payments. Option 2: 12 monthly payments by Direct Debit. - Plan without Indorex household spray for fleas and house mites. Option 3: discounted single prepayment equivalent to 11 months payments. Option 4: 12 monthly payments by Direct Debit. Terms and conditions: 1 - there is a discount of 5% on your total monthly fee if you have more than one pet on a Pet Health Plan. 2 - there is a joining fee of £10 per owner (for life), plus a pet joining fee of £5 per animal. These fees will be collected with the first payment. 3 - If you cancel you plan at any time and the value of the treatments received exceeds the payments you have made, you may be required to pay the difference to the practice. 4 - No refunds are payable for any months paid before a pet's death except at the discretion of the practice. 5 - A loyalty card on which all records will be kept will be kept at our end for each pet.
